I acknowledge that in order to avoid being charged on the 1st of the upcoming month, my cancellation must be submitted in writing by 11:59 PM on the 20th of the current month. I understand this form is required, as all cancellations must be done in writing.

Cancellations received after the 20th will not take effect until the following month.
Example: If you cancel at 12:00 AM on June 21, you’ll still be billed for July, and your membership will officially end on July 31.

Cancellations submitted during the same month as sign up will become effective the following month. For instance, if a member joins in June and cancels within that month, the cancellation will be processed in July. As a result, the member will be billed for their standard membership on July 1 and have access to services through July 31. Cancelation will take effect August 1st.

If charges continue after your cancellation, Glo Tanning must be notified within 90 days. A copy of your written cancellation will be required to review and dispute any charges.

By completing this form, I am formally requesting to cancel my Glo Tanning membership. I understand that if the cancellation guidelines are not followed, I will still be billed for the next month’s dues.

You’ll receive a confirmation email from our customer service team within 1–2 business days after your request is reviewed. Please keep a copy of that email for your records.

By submitting this form, I am officially requesting to cancel my membership. I acknowledge that if I do not meet the cancellation requirements by the deadline, I will be charged the regular monthly dues for the following month. I understand that I must submit my cancellation request in writing via this form and that my membership must be canceled by 11:59 PM MST on the 20th of the month to avoid being charged on the first day of the following month. If I cancel after the 20th, my billing will continue for one additional month, after which my account will be fully closed. I acknowledge that if my account is currently frozen and I choose to cancel, I will forfeit my freeze credits. If my billing continues after I have canceled, I must notify Glo Tanning within 90 days. I understand that I will need to provide a copy of my written cancellation request to dispute any charges.
