Starting next month, my account will be charged a $5 monthly freeze fee until I decide to reactivate my membership.
To ensure the freeze begins on the 1st of the upcoming month, requests must be submitted no later than 5:00 PM on the final day of the current month.
Please Note: Freeze requests submitted during your sign-up month will take effect the following month. For example, if you join and request a Freeze in June, you’ll still pay your regular membership fee on July 1 and have access through the end of July. Starting August 1, your freeze will take effect, you’ll be charged $5 per month until you unfreeze.
If a freeze request is submitted after the 1st, the regular membership charge will still apply for that month, and the freeze will begin the following month. I’ll still have access to tanning through the remainder of the current month after submitting my request.
Upon the activation of my freeze, the monthly fees of $5 will be incorporated into my in-store credit at a rate of $10 per month. This credit can be used toward products and services. It is nonrefundable, cannot be applied to monthly dues, and will be forfeited if I cancel my membership before unfreezing.
By submitting this form, I am requesting that my membership be placed on hold effective the 1st of next month.
